1. Your Name
2. Contact Email used for both the AMKE forums (the one used to sign up on here) and if you have a AMKE email address (I need both)
3. User Name on these forums
4. Positions you hold with the convention


The forums have seen a few changes and I need to make sure I have everyone's correct info. Thanks in advance!
